<p>This patient is s/p Retinal Detachment Repair x 2 with Silicone Oil Use on second case and ultimate removal. Developed Dense White Cataract which is clearly visible in this unedited video. Dr. Todd Fladen of The Fladen Eye Center demonstrates his method of removal, assisted with Vision Blue. As the patient had 4.5 diopters of Astigmatism, a Toric Astigmatic IOL Alcon SN6AT5 was inserted to reduce the astigmatism by 50%. Larger diopter Toric IOL, ie T9 is not available in USA; would have completely eliminated Astigmatism. The cataract was fortunately very soft and only Irrigation and Aspiration was required with phacoemulsification. A dense white posterior capsular plaque remained at end of procedure which could not be aspirated. This will be lasered with a YAG laser at 90 days postop to reduce likelihood of post laser retina edema and recurrent Retina Detachment. Patient was CF preop and reported 20/200 vision on Day 1. Vision anticipated to improve to 20/100 or better after YAG laser.</p>

As a normal part of aging, the lens begins to cloud and causes a gradual, painless loss in vision. Cataract removal is most often performed to better examine the back of the eye when monitoring for damage from certain diseases such as diabetes or glaucoma and to improve vision. There are two main types of cataract removal. The large majority of cataract surgeries are performed using the phacoemulsification technique. During the phacoemulsification technique an ultrasound probe breaks the cloudy lens into tiny fragments. The fragments are vacuumed out through a tiny incision. An intraocular lens implant is then inserted to replace the natural lens that was removed. Because the incision is tiny, stitches are often not necessary and visual improvement is usually noted relatively soon after surgery. During the extracapsular technique the cataract is removed as one entire piece. This requires a larger incision and stitches.
